Key Responsibilities
The nurse is responsible for the assessment, analysis, planning and facilitation of healthcare services for designated members on behalf of our clients.
The Nurse/CM provides on call telephonic coverage and communication with patients who are seeking physician access and advice through the TRUDOC 24X7 physician call center, assisting the physician with coordination of care.
This position works collaboratively with internal and external stakeholders including physicians, payers, hospitalists, Nurse/CM and patients/ caregivers to identify barriers and risks, share information, develop intervention plans and advocate for the needs of patients to improve healthcare outcomes and quality of care.
Skills and attributes for Success
Strong interpersonal skills and working knowledge of organization and healthcare system processes
Computer proficiency including knowledge of word processing and principles of databases
Ability to work with a diverse member population, including culturally and economically disadvantaged communities
Understands medical record keeping requirements and maintain strict confidentiality/privacy requirements
Ability to adhere to a flexible work schedule including some weekends, nights, and holidays.
Perform other duties and responsibilities as assigned
Carry out nursing procedures and treatment (giving meds, withdrawing blood samples, monitoring patients during Company Visits, Home Visits and Client Engagement programs).
Home Visits with / without Doctors for John Hopkins Hospital at Home Program.
Assumes accountability for delegation of case management and care coordination.
Maintain and respond to collected data from emails, patient’s EMR, and portal in an accurate, meaningful and timely manner.
Shares and documents findings accurately and in a timely manner and maintain internal patient trackers.
Analyzes screening and referral data to determine proper case management, care access and care coordination.
To qualify for the role, you must have:
Unrestricted active RN license in DHA
Minimum of 3 years’ clinical experience
Additional Skills and requirements (if required):
Knowledge of utilization and case management
Multi-lingual abilities are a plus
